How Do I Apply for WIC?

Make an Appointment

Call 636-789-2686 Option 5 to schedule an appointment

Bring the following
  • Each family member applying must be present.
  • One proof of income:
    • Proof of current program participation:
      • MO Health Net letter or card
      • SNAP benefit letter
      • TANF benefit letter
    • Paystubs (all household members, last 30 days)
    • Unemployment benefit letter
    • Social Security benefit letter
    • Alimony or Child Support letter
    • Most recent tax return or W-2 form
  • One proof of address (where you live):
    • Bill mailed within the last 30 days
    • Paystub with name and address (last 30 days)
    • Mortgage statement
    • Rent receipt
    • Property tax receipt
    • Voter registration card
    • Letter from Mo Health Net, SNAP, TANF
    • Signed letter from church, shelter, employer that has the address written on it.
  • One proof of identity for each person applying:
    Social Security Cards NOT Accepted
    • Photo ID for an adult:
      • Driver's license or State ID
      • Employment, Military, or School ID
      • Passport
    • ID for infant or child:
      • Medicaid or Insurance card
      • Letter from Mo Health Net, SNAP, TANF
      • Immunization record
      • Birth certificate
      • Crib card, discharge papers, hospital band
  • Foster parents: bring custody papers.
